POSITION SUMMARY | Under the supervision of the pharmacy manager, provides patient care, performs clinical tasks, oversight of dispensing prescriptions and provides patient consultations within the scope of a California licensed pharmacist to manage medication therapy and ensure optimal patient health and wellbeing. Complies with state and federal laws, rules, regulations, and organizational policy and procedures. Collaborates with health care providers to provide appropriate pharmacological guidance while resolving prescription ambiguities and issues. Provides direct supervision and responsible for duties performed by pharmacy technicians. Maintain security of pharmacy, appropriate storage, handling, ordering and preparation of drugs within a safe and clean work environment. | | | |

Key Responsibilities and Essential Functions | Function | Responsibilities | % Time | Drug Distribution | Reviews, interprets and dispenses medications by licensed prescriber orders as required by state and federal agencies. Reviews patient profiles to ensure appropriate medication therapy and dosage. Screen all drug, allergy and disease interactions, contraindications, adverse interactions and review therapeutic duplications. Dispenses medications by compounding, packaging, and labeling. Monitor drug therapies and advising interventions. Prescribes and administers selected medications per protocol. | 45% | Counseling/Consultation | Provides pharmacological information to patients and clinical staff. Communicates with Health Care prescribers to resolve prescription problems. Counsels patients on proper use, possible interactions and all aspects of medications / over-the-counter products. Review compliance, utilization pattern, and response to therapy using standard outcome criteria. | 25% | Pharmacy Operations | Responsible for direct supervision of duties performed by pharmacy technician. Complies with State and Federal drug laws, rules and regulations. Remove outdated drugs from the pharmacy inventory, reviewing pharmacy and clinic recall medications. Acknowledge current licensure dates. Maintain pharmacy security during business hours. Reviewing and advising on existing, new and developing pharmacy legislation. Oversee pharmacy operation in absence of PIC and Staff Pharmacist. | 15% | Administrative, Information, and Research Duties | Appropriately identifies and resolves all pharmacy-related problems. Ensures most efficacious, safe and cost-effective medications are used. Identify and provide input to improve policy/procedures. Stay up to date with current clinical guidelines and drugs. Maintains accurate pharmacy management system. Participates in committees. Protects patients and technicians by adhering to infection control and Hazardous Drug protocols. Maintain safe and clean working environment. Interpret drug information resources. | 10% | Record Maintenance | Maintain non-controlled and controlled substance (CS) inventory and assist with quarterly inventory reconciliation and biennial CS inventory. Identify and report CS discrepancies to manager and file incident report. Maintain records for CS and prescription records per DSCSA. Maintain pharmacy records accurately and for appropriate time period. Performs Quality Assurance audits and document medication errors in database and to manager.
