POSITION SUMMARY

Under supervision of the pharmacy manager, provide the full range of distributive, clinical, consultative, research, and/or administrative pharmaceutical services in an ambulatory care setting.  Perform other related duties as required and assigned by the pharmacy manager including, but not limited to prescribe under clinic protocol, monitor the use of selected classes of medications, participate in clinic conferences, orient clinic professionals regarding pharmaceutical services, and precept pharmacy students. Select, purchase, compound, dispense, and assure proper storage of medications and other therapeutic agents consistent with organizational policies and procedures, community standards of practice, and state and federal regulations. Serve as a member and a consultant to other licensed healthcare professionals in the clinic; provide pharmaceutical care to clinic patients; disseminate drug information to clinic professional staff, and participate in educational programs as required to meet organizational needs. Participate in pharmacy administrative projects and provide supervision to pharmacy technician staff and clerical staff.

B-1 Appropriately identifies and resolves problems including, but  not limited to prescribing errors, formulary compliance, patient               customer service complaints, third party billing errors,               controlled substance accountability, and adherence to               organization policies and procedures. (E) B-2              Ensures that the practice dictates the most efficacious, safe               and cost effective medications are used when delivering               pharmaceutical care to the patient population. (E) B-3              Identifies opportunities for improvement as part of continuous               quality improvement addressing item A-1. (E) B-4              Provides input regarding the development and implementation of  deparment policies and procedures. (M) B-5              Acts as pharmacy liaison in generating outreach to the clinical               staff, e.g., in-service programs. (M) B-6              Identifies opportunities to improve medication safety;               demonstrates competency in documenting medication errors in               the pharmacy quality assurance database. (E) B-7              Helps maintain the pharmacy reference collection, policy &               procedure manual, and information system operations manual. (M) B-8              Assists in maintaining the controlled substance perpetual               inventory, conducts audits as per policy and procedure and               performs the biennial controlled substance inventory. The               incumbent identifies controlled substance discrepancies,                reconciles discrepancies and reports the results to the               manager. (M) B-9              Ensures that pharmacy records are maintained accurately and               for the appropriate time period as required by regulatory               standards and third party payors. (M) B-10              Maintains the pharmacy information system  patient,               medication, and prescriber databases. (M) B-11              Assists in maintaining appropriate inventory, adjusts par               levels after reviewing utilization reports and insures inventory               prices are accurately reflected in the pharmacy information               system medication database. (M) B-12              Assists in insuring that all third party claims adjudicate;               assisting resolving all third party billing errors, serves as a               billing resource for other pharmacy staff  members, and               communicates unresolved billing issues to the manager in a               timely fashion. (M) B-13              Manages the mind-body herbal formulary and serves a               resource for other pharmacy staff regarding appropriate use               and potential drug-herbal interactions. (M) B-14              Coordinates the pharmacy emergency contraceptive               prescribing program,               performs quality assurance audits and               reports results to the               pharmacy manager; serves a staff resource and liaison with the               clinic nursing and medical staff. (M) B-15              Serves as a liaison to the organization's clinical committees as               assigned by the pharmacy manager. (M) B-16              Provides supervision of licensed technical staff; communicates            positive and negative job performance to the pharmacy manager while maintaining confidentiality. (E)

Drug Distribution

C-1 Selects and dispenses medications as required to fulfill  prescriber's orders. (E) C-2 Maintains accurate patient medication profiles in the pharmacy               information system. (E) C-3              Reviews the prescription before dispensing to insure correct               billing. (E) C-4              Communicates with prescribers and other allied health               professionals to resolve potential or actual prescription               problems. (E)

Clinical Activities

A-1 Reviews patients medication profiles as required to insure  appropriate medication therapy, appropriate dosage, screen for               drug-drug interactions, screen for drug-nutritional supplement               interactions,  screen for drug-disease state interactions, review               age-specific medication use considerations,  and therapeutic               duplication. (E) A-2              Provides drug information and medication information to               patients and clinical staff. (E) A-3              Assesses appropriateness of refill requests taking into account               compliance and response to therapy using standard outcome               criteria. (E) A-4              Counsels patients on proper use of medications. (E) A-5              Prescribes selected medications under protocol to patients               including, but not limited to emergency contraception. (E) A-6              Precepts intern pharmacists/pharmacy students as assigned to               the site. (M) A-7  Administers immunizations as specified by the organization and as permitted by scope of practice and regulation (E)
